Utilización de la oxigenoterapia hiperbárica en la curación de heridas. Revisión bibliográfica

Resumen

Objetivo: Conocer el uso de la oxigenoterapia hiperbárica en el tratamiento de heridas. 

Metodología: Se realizó una revisión bibliográfica narrativa en las principales bases de datos (PubMed, SciELO, Dialnet, CUIDEN) y en dos metabuscadores, Google Académico y Mergullador, limitando la búsqueda en los últimos 5 años y filtrando aquellos artículos que versaran sobre tratamiento en humanos. La selección de documentos se realizó basándonos en criterios de inclusión y lectura crítica. 

Resultados: Se localizaron un total de 1.223 artículos, 45 cumplían criterios de inclusión. Tras una lectura detallada se revisaron 14 artículos (5 ensayos clínicos, 1 revisión sistemática, 3 casos clínicos, 2 revisiones de la literatura no sistemáticas y 3 estudios observacionales). La oxigenoterapia hiperbárica posee acción antiinflamatoria y reduce el componente edematoso presente en muchas de las lesiones. No es concluyente si hay disminución del riesgo de amputación por uso de esta terapia. 

Conclusiones: La oxigenoterapia hiperbárica se consolida como una terapia segura y fiable, tanto de lesiones agudas como crónicas. El acceso a este tipo de tratamientos continúa restringido a patologías más avanzadas e importantes por su elevado coste. Consideramos como líneas de investigación futuras el abordaje cualitativo de este problema para tratar de conocer cuáles son las experiencias de los usuarios de este tratamiento.

ABSTRACT:

Objective: To know the use of hyperbaric oxygen therapy in the treatment of wounds. 

Methodology: A narrative literature review was carried out in the main databases (PubMed, SciELO, Dialnet, CUIDEN) and in two metasearch engines, Google Scholar and Mergullador, limiting the search in the last 5 years and filtering those articles that They will deal with treatment in humans. The selection of documents was made based on criteria for inclusion and critical reading. 

Results: 1,223 articles were located, 45 met inclusion criteria. After a detailed reading, 14 articles were reviewed (five clinical trials, one systematic review, three clinical cases, two non-systematic literature reviews and three observational studies). Hyperbaric oxygen therapy has anti-inflammatory action and reduces the edematous component present in many of the lesions. It is not conclusive if there is a decrease in the risk of amputation due to the use of this therapy. 

Conclusions: Hyperbaric oxygen therapy is a safe and reliable therapy for both acute and chronic lesions. Access to this type of treatment continues to be restricted to those most advanced and important diseases due to its high cost. Consider as qualitative lines of research the qualitative approach to this problem to try to know what the experiences of the users of this treatment are.
