VITAMINA B12 Y CICATRIZACIÓN DE HERIDAS: REVISIÓN EXPLORATORIA

OBJETIVO. El objetivo de este trabajo es analizar la
evidencia científica disponible sobre el efecto del déficit de
cobalamina en la cicatrización de las heridas.
METODOLOGÍA. Revisión exploratoria sobre el efecto del
déficit de cobalamina en la cicatrización de las heridas
mediante el análisis de trabajos sobre el tema indicado en
las principales bases de datos. El estudio realizó la
búsqueda y selección de artículos, analizando
posteriormente la información relevante sobre el tema
planteado. RESULTADOS. La revisión bibliográfica
realizada identificó 8 artículos provenientes de diferentes
países y ámbitos de atención, que abordaban diferentes
tipos de heridas: pies diabéticos (50%), heridas
postquirúrgicas (25%), lesión por presión (12,5%) y
queratosis actínicas (12,5%). Algunos estudios observaron
que el déficit de cobalamina incrementa la prevalencia de
úlceras de pie diabético (p<0,010) y las amputaciones
(p=0,029), prolonga los tiempos para la cicatrización
(p<0,001), se asocia a un aumento de las complicaciones
tras cirugía reparadora por colgajo en lesión por presión
(p=0,005), así como las diástasis (p=0,008), cicatrices
inestéticas (p<0,001) e incómodas/molestas (p<0,001) en
las heridas postquirúrgicas.
CONCLUSIONES. La evidencia disponible sobre la
asociación entre los niveles bajos de cobalamina y la
cicatrización es limitada e inconsistente, siendo necesario
desarrollar estudios de investigación que aporten una
evidencia más homogénea y consistente. Mientras tanto,
las decisiones clínicas deben tomarse e interpretarse con
precaución, debiendo considerar cuidadosamente la poca
evidencia disponible y sus limitaciones.
