Úlcera recalcitrante por calcifilaxia: evolución comparativa entre injerto autólogo en sello y microinjertos autólogos de piel

[Recalcitrant calciphylaxis ulcers: a comparative evolution between autologous pinch grafts and skin micrografts]

Resumen

Se presenta el caso clínico de una paciente con calcifilaxis cutánea bilateral y úlceras recalcitrantes en miembros inferiores, tratada mediante dos estrategias regenerativas autólogas: injertos en sello y microinjertos de piel mediante tecnología Hy-Tissue Micrograft® (HT-MG). El manejo sistémico (tiosulfato sódico intravenoso, analgesia, control metabólico y compresión terapéutica) se mantuvo constante durante el seguimiento. La extremidad izquierda, tratada con injertos en sello, mostró una evolución tórpida con episodios infecciosos y dolor persistente, alcanzando la epitelización completa a los seis meses. Por el contrario, la extremidad derecha —tras el fracaso del injerto en sello y presentar exposición tendinosa— fue tratada con HT-MG previo desbridamiento quirúrgico. Se observó una rápida granulación, remisión precoz del dolor y cicatrización completa en diez semanas. Este caso sugiere que los microinjertos autólogos en suspensión podrían ofrecer ventajas clínicas significativas en el tratamiento de úlceras por calcifilaxis  refractarias.

 ABSTRACT:

 We present a clinical case of a patient with bilateral cutaneous calciphylaxis and recalcitrant lower limb ulcers treated with two different autologous regenerative strategies: pinch grafts and skin micrografts using Hy-Tissue Micrograft® (HT-MG) technology. Systemic management, including intravenous sodium thiosulfate, analgesia, metabolic control, and therapeutic compression, remained constant throughout the follow-up period. The left limb, treated with pinch grafts, showed a torpid progression with intercurrent infectious episodes and persistent pain, achieving complete epithelialization after approximately six months. In contrast, the right limb—following the failure of initial pinch grafting and presenting tendon exposure—was treated with HT-MG after surgical debridement. This approach resulted in rapid granulation tissue formation, early pain relief, and complete healing within ten weeks. This case suggests that autologous suspension micrografts may offer significant clinical advantages in the management of refractory calciphylaxis ulcers.
