Abordaje postquirúrgico de un caso de gangrena de Fournier desde atención primaria

[Post-surgical approach to a fournier gangrene case from primary care]

Resumen

Presentamos el caso de un varón con gangrena de Fournier en zona peneal y testicular, que es intervenido quirúrgicamente para desbridamiento de tejido necrótico y posteriormente dado de alta para seguimiento de curas desde su centro de salud. Iniciamos un plan de tratamiento que incluye desbridamiento de detritus, control de la carga bacteriana, fomento del tejido de granulación y educación para la salud. Los resultados muestran, a través de imágenes, la evolución del caso con la descripción de las curas realizadas, que facilitó una posterior reconstrucción plástica de la zona lesionada. Las descargas bacterianas con apósitos con Cloruro de Diaquilcarbamilo (DACC) y plata, junto la desbridación de la biopelícula con fomentos de Polihexanida (PHMB) y la utilización de apósitos moduladores de las metaloproteasas y de colágeno bovino, facilitaron la resolución del caso y mejoró la calidad de vida del paciente durante el proceso.

 ABSTRACT:

We present the case of a man with Fournier gangrene in the penile and testicular area, who underwent surgery to debride necrotic tissue and then discharged to follow up on cures from his health center. We initiate a treatment plan that includes debridement of debris, control of bacterial load, promotion of granulation tissue and education for health. The results show, through  images, the evolution of the case with the description of the cures carried out, which facilitated a later plastic reconstruction of the injured area. The bacterial discharges with DACC and silver dressings, along with the debridement of the biofilm with PHMB fomentations and the use of modulatory dressings of the metalloproteases and bovine collagen, facilitated the resolution of the case and improved the quality of life of the patient during the process.
